Home Assessment Report
Program Description
Maple Tree Consulting and Support Service’s (MTC) Home Assessment Report program collaborates with Children and Family Services and Collaborative Service Delivery agencies to create accurate and reliable SAFE Home Assessment Reports (HAR). While working in partnership with Children and Family Services and Community Stakeholders, MTC evaluates applicants required to complete a home assessment. The HAR provides a recommendation based on the applicant’s family’s willingness, readiness, and ability to parent effectively.
Program Summary
- MTC’s Assessors are trained and certified as SAFE Assessors.
- Experienced MTC SAFE Assessors and supervisors work collaboratively with interdisciplinary teams.
- Case conference and STM availability from assigned MTC SAFE Assessors and supervisors.
- SAFE Assessors act as an independent source to determine the placement viability of applicants.
