Program Description

Maple Tree’s Youth Work Program understands that youth who have experienced involvement with Children’s Services are often disconnected from their families, peers, culture, and communities. Our Youth Workers collaborate with Children’s Services, other agencies and stakeholders to engage with youth, encouraging them to enrich their relationships, grow their support network, and connect with their communities, thereby enhancing their sense on belonging and attachment. Through healthy relationships, mentoring, and relentless support, Maple Tree Youth Workers facilitate opportunities for youth to increase their capacity and build life skills necessary for a successful transition to adulthood. For some youth, this may include education, employment, volunteerism, money management, or accessing affordable housing.     

Program Summary: Mentorship Stream

  • Access to skilled Youth Workers with experience supporting diverse populations and multidisciplinary teams
  • 1-to-1 scheduled youth worker support in placement or in the community
  • Positive behavioural role modeling and relationship building
  • Life skill coaching and development
  • Youth Worker attendance at STM and case conferences
  • Quarterly reports provided to service team and contact notes following each visit

Program Summary: At Risk Stream

  • Access to skilled Youth Workers with experience supporting at risk youth and multidisciplinary teams
  • 1-to-1 youth worker support for hard-to-reach youth from a trauma informed framework
  • Goal setting in alignment with current Transition to Independence Plan
  • Positive behavioural role modelling and relationship building
  • Meeting the youth where they are at, attending medical or legal appointments, relentless outreach
  • Youth Worker attendance at STM and case conferences
  • Quarterly reports provided to service team and contact notes following each visit
